I understand that the risk of becoming exposed to and/or infected by the Corona virus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, agency staff, and other agency clients and their families.

I voluntarily attend inspections conducted by Century 21 The Paramount Group and acknowledge that I am increasing my risk of exposure to Corona virus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment. This is to reduce the spread and keep all Century 21 The Paramount Group' clients, staff and local community safe.

In attending my appointment I confirm:

- I do not have symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
- I have not travelled internationally or interstate within the last 14 days.
- I (knowingly) have not been exposed to someone with a suspected and/or confirmed case of the Corona virus/COVID-19.
- I am not awaiting results from a test for Corona virus/Covid-19 that has not yet been cleared as non contagious by state or local public health authorities.
- I have not visited any of the current Corona virus/Covid-19 hot spots.
